If you have a good night time diaper, you may not need an extra layer inside the soaker. It all depends on what materials you use to make the soaker. If you have a heavy wetter, I'd suggest an extra layer in the wet zone. I haven't found the need for the extra layers unless the fleece or wool is really thin.
